Your drug list, Blue Rx Value™, tells you if certain drugs require approval before they can be covered by your plan and any quantity limits that might apply.
Drugs indicated as non-formulary cannot be approved through the prior authorization process. Obtaining the approval in advance will help to prevent delays at the pharmacy. If the drug is approved, any in-network pharmacy can fill the prescription.
Your doctor can request approval using the Global Prior Authorization Form.
